Connected Community Care

Role: Co-ordinates GP Referrals to link workers who meet up with the person referred to Social Prescribing and draw up a jointly agreed programme to meet that person’s needs

Role: Supporting families in North Belfast, providing Nutrition and Exercise Programmes, Social Media Awareness, Stress Management Clinics , Young peoples programmes etc

LOVEWORKS

Role: A Belfast-based, worker-owned cooperative. Generates dignity and wellbeing for its workers who have gained skills and experience in gardening, bike repair and baking whilst providing a useful service to the community.
